As a participant of one of the medical plans offered by CHW, you enjoy the highest
level of benefits and the lowest out-of-pocket expenses when you receive care from
network physicians nationwide. As a member, you can choose either in-network
or non-network benefits. In-network benefits feature co-payments for
primary and specialty care.
The plans provide access to a large network of providers through the Schaller Anderson Healthcare Network and the First Health
national provider network for care outside of Arizona. Go to the Provider Directory link on this Web site for more information.
The medical plans also include coverage of prescription medications based on a Preferred
Drug List (PDL), which is a list of preferred drugs covered under this health plan.
The PDL includes brand name drugs as well as generic drugs approved by the Food
and Drug Administration (FDA). Generic drugs must meet the same standards for safety
and effectiveness as brand name drugs.
There are separate co-payments for generic versus brand name drugs that are listed
on the PDL, as well as drugs that are not listed. Some drugs require pre-certification
before they are covered by the plan.
For more information about specific coverage and co-payment amounts, see
A Guide to Your 2008 Medical Benefits.
CHW medical plans encourage you to discuss your medication needs with your prescribing
physician, as well as your pharmacist. They can help you determine the most appropriate,
cost-effective medication for your individual needs.
